Essential Functions

To perform clinical duties under direct supervision of a provider/ physician assisting with patient care. The LVN will work with provider and additional clinical support staff to maintain clin flow. LVN will always maintain confidentiality (HIPAA).

Job Summary

Our thriving Family Medical Practice needs an experienced Licensed Vocational Nurse whose responsibility is to work with Provider to coordinate patient centered care. The LVN will work as a team to maintain clinic flow of all patients. LVN duties include but not limited to: Obtain detail medical history, vital signs, medication review of patients, performing laboratory services/EKG/other procedures under supervision as needed, telephone messages and administration of medication/immunizations. We’re looking for a professional who believes that patients should be treated as people rather than numbers in a file, and who understands the value of compassionate service. The job includes significant amounts of multi-tasking, but the ideal candidate will know when to slow down, look patients in the eye and provide personalized service. Roles and Responsibilities

  • Escort Patients to Exam Rooms, Interview Patients, Measure Vital Signs, and Document all Information in Patient’s Chart.
  • Assisting the Physician/Provider During Examinations
  • Medication/Immunization administration
  • Explaining Medical Procedures to Patients
  • Calling in Prescriptions to Pharmacies
  • Signing out sample medication according to provider orders
  • Logging medications received from assistance programs (create TE and notify patient via phone)
  • Collect Laboratory Specimens (phlebotomy, Urine Specimens, Glucose, Strep/Flu swabs)
  • Sterilize Medical Instruments
  • Keep Exam Rooms Stocked with Adequate Medical Supplies
  • Notify appropriate personal when supplies need to be ordered to maintain clinic flow
  • Take Telephone Messages and Provide Feedback and Answers to Patient/Physician/Provider/Pharmacy Calls
  • Triage and Process Messages from Patients and Front Office Staff to Physician/Provider
  • Maintain all Logs and Required Checks (i.e. refrigerator temperatures, emergency medications, expired medications, oxygen, immunizations, etc.)
  • Changing Biohazard Trash/sharp boxes as Needed
  • Clean Exam Rooms After each Patient

Qualifications and Education Requirements

  • High School Diploma
  • Graduate from an approved nursing program. Nursing licensure in the State of Texas

Medical Assistant Job Opening

Position: Medical Assistant

Reports to: Nurse Manager and Director of Operations

Administers patient care to designated patients under the direction of the Nurse Practitioner or Physician. Performs a variety of indirect patient care duties. Participates in the planning and evaluation of patient care and patient education.

Position Requirements

  • High School diploma or equivalent required. Current Certified Medical Assistant.
  • Must have basic clinic or medical office experience. Bilingual preferred but not required.
  • Computer and office equipment knowledge.

Restrictions

May not undertake any procedure not trained in.

Duties and Responsibilities

  • Interview patients to obtain medical information, collect vital signs, weight, and height.
  • Show patients to examination rooms and prepare them for the provider.
  • Record patient’s medical history, vital statistics, mediation and information such as test results and medical information and records.
  • Prepare and administer medications as directed by the provider.
  • Collect blood, tissue, or other laboratory specimens, log the specimens, and prepare them for testing.
  • Explain treatment, procedures, diets, and any instructions under the direction of the provider.
  • Assist the provider to examine and treat patients; prepare instruments and materials.
  • Prepare treatment rooms for patient examinations, keeping the room neat and clean.
  • Sterilize instruments and dispose of contaminated materials.
  • Schedule appointments for patients regarding follow up and referrals.
  • Perform general office duties such as answering telephones and documenting patient calls and messages.
  • Perform routine laboratory tests and sample analyses; prepare samples for retrieval from outside lab services.
  • Operate EKG and other medical equipment as directed by a provider.
  • Is familiar with the Patient Centered Medical Home (PCMH) model of care.
  •  Works with provider/care team to ensure appropriate PCMH documentation is completed in medical charts such as care management goals/follow-up, quality measures, and care coordination efforts.

The Dentist is responsible for providing clinical dental services and preventive dental care to patients.

Position Requirements

  • DMD or DDS from an accredited program

Position: Nurse Manager

Reports to: Chief Executive Officer

Under the direction of the CEO, the Nurse Manager is responsible for overseeing clinical support services, risk management, and quality health care deliver in the clinic setting. The Nurse Manager also participates on the leadership team.

Position Requirements

  • Experience with Physician and Office Management
  • 3-5 years of experience preferred
  • Knowledge of personnel administration procedures and practices as well as principles, techniques, terminology and procedures of sound management
  • Knowledge of health care administration practices
  • Skill in verbal and written communications
  • Ability to work effectively with staff, public, and external agencies
  • Demonstration and certification of clinical skills
  • License as a LVN or RN in the state of Texas

Duties and Responsibilities

  • Reports to the CEO and attends all required leadership and quality meetings.
  • Supervises medical assistants and other nurses (RN, LVN). 
  • Plans, organizes, directs, coordinates, controls, and evaluates the clinical support team and efficiency in each clinic.
  • Serves as a part of the leadership team and Quality Assurance/Quality Improvement committee.
  • Helps establish/implement goals, objectives, policies, procedures, and systems for the health center and is responsible for the development and implementation of these plans
  • Coordinates and oversees the Risk Management plan of the health center, under the direction of the QA/QI Committee and the CEO.
  • Evaluates performance and recommends merit increases, promotions, and disciplinary actions for medical assistants and other nurses (RN, LVN). 
  • Works with medical staff to ensure quality patient care and services are provided
  • Is familiar with the Patient Centered Medical Home (PCMH) model and helps to set, achieve, and maintain goals related to PCMH recognition. 
  • Trains medical staff on the appropriate documentation of PCMH required activities, especially in the documentation of care management (goals and follow-up), quality measures, and care coordination.
  • Works with Director of Operations and supports medical providers to provide same-day access for all patients. 
  • Hires medical assistants and nurses (under the direction of the Director of Operations and CEO). 
  • Practices and adheres to the “Code of Conduct” and “Mission Statement”
  • Acts as an advocate for underserved individuals with sensitivity to each client’s unique needs
  • Maintains clinical standards in the clinics through appropriate evaluation of clinical support staff

A typical dental assistant should have basic knowledge of the basic body systems and functions, oral anatomy, dental science, dental materials, computer and office applications, and oral radiology. Dental assistant are required to carry RDA or CDA license obtained in the State of Texas.

Patient care responsibilities for Dental Assistants

  • Prepare patients for treatment. Retrieve patient’s’ dental records.
  • Hand dental instruments, materials, and equipment to the dental provider.
  • Use suction or similar devices to help keep patients’ mouths dry and clear.
  • Prepare tray setups for dental procedures.
  • Sterilize and disinfect instruments and equipment.
  • Prepare materials for making dental impressions and restorations.
  • Offer patients instruction in postoperative steps and general oral healthcare.

With additional X-ray certification: Expose radiographs and process dental X-rays as directed by a dentist.

Laboratory Duties for Dental Assistants

  • Check on inventory including disposables every week
  • Check on sterilization protocols                                                 
  • Check on ultrasonic and autoclave functioning
  • Check on instruments and make sure instruments are properly sterilized and maintained.
  • Maintain log of handpieces sent out for repair
  • Maintain sterilization log and present the log when requested
  • Make temporary crowns.

Office Duties for Dental Assistants Include the Following

  • Greet and receive patients.
  • Maintain record such as consents, health histories of patients. Schedule and confirm patients’ appointments.

In addition to above duties dental assistants are expected to perform a variety of assignments and duties that have varied levels of responsibility during a typical day on the job.

DO’s

  • Show up at least 15 minutes before start time Mandatory
  • Check with front desk before bringing the patient to the dental chair
  • Check to see if patient has signed the general consent
  • Do need to check on health history to see if they are current
  • Do make sure that consent for treatment is obtained after the exam, if you are busy let the checkout person know that consent needs to be obtained.
  • Do need to run Texas prescription monitoring before printing prescriptions
  • Do send lab cases on same day if possible

DONT’s

  • Don’t say anything to patient or over promise unless you talk to the dental director or provider
  • Don’t let patient leave without making future appointment with check out desk Don ‘t bring patients to the dental chair if there are other treatment patients
  • Do not add a pop up or comm log entry without talking to the Dental Director. Do not make patient ready without collecting payment
  • Do not make appointments for a patient if they have a balance unless prior balance is paid
  • or patient is willing to pay their past balances.
  • Do not use private email or Gmail or yahoo mail to send patient documents or referrals. Use your crossroads family care email.

DOCUMENT PRESERVATION: Remember patient casts and other lab work need to be preserved for 5 years from date of service and therefore cannot be trashed All of them need to be stored and packed into boxes and old boxes need to be moved to the storage.
