Crossroads family care accepts Medicare, Medicaid, and most insurance plans. In addition, Crossroads provides programs to assist patients with affordable health care.

We help individuals and families with choosing and enrolling in Health Insurance Marketplace plans (during open enrollment) that best fit their needs. We also assist with applications for programs such as Medicaid, Food Stamps, TANF, CHIP or simple renewals, and with choosing medical plans, primary care physicians, dental plans and primary dentists after enrollment.

Sliding Fee Scale – Patients without insurance may qualify for reduced-cost healthcare based on monthly income. Patients may request an application at the front desk or click here to download the form.

Primary Health Care – Patients without insurance may qualify for this state-funded program to receive primary care services at a minimal cost based on monthly income. The program is for adults and covers wellness exams, well woman exams, sick visits, and chronic disease management including lab work. Title V Child Health and Dental – Patients ages 21 and under may qualify for this state-funded program to receive wellness exams at a minimal cost based on monthly income. Qualification for any of the above programs is dependent on income. Patients will need to provide proof of income and residency as well as household size.
